Iron Point Home Health Care, Inc. have openings for field-based, California-licensed Physical Therapist, to work as a home visiting PT and join our team in. The ideal candidate continually strives in providing exceptional patient care services; experienced in providing comprehensive full-body medical assessments and treatments, clinical charting, possesses exceptional interpersonal and communication skills.
Physical Therapists provide services that help restore function, improve mobility, relieve pain and prevent permanent physical disabilities of patients suffering from injuries or disease. Restore, maintain, and promote overall fitness and health. Their patients include accident victims and individuals with disabling conditions such as low back pain, arthritis, heart disease, fractures, head injuries, and cerebral palsy.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment.
· Responsible for planning, evaluation, and treatment implementation of assigned caseload.
· Preparing clinical progress notes.
· Supervising therapy assistants as appropriate.
· Evaluating outcomes.
· Educate patients and/or families about proper safe use and recommend methods for installation of adaptive equipment.
Communication
- Prepares clinical notes and updates the primary Physician when necessary.
- Communicates with the Physician and Physical Therapist Assistant regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives Physician’s orders as required.
- Communicates with community health related persons to coordinate the care plan.
Additional Duties
- Participates in on-call duties as defined by the on-call policy.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Instructs, supervises and evaluates Physical Therapy Assistant every week and as needed.
- Adhere to all Company policies and procedures.
- Demonstrate flexible and efficient time management techniques.
- Prioritize workload so as to meet productivity standards and applicable deadlines.
- Communicate with Company Administrator on a regular basis about project status and issues.
- Report to work on time and prepared to perform duties of position.
Position RequirementsMust be currently licensed to practice as a Physical Therapist, licensed by the state.
- New graduate welcome - PAID TRAINING
- Maintains a current CPR certification.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Must be able to travel to prospective patients’ residences.
- Self-directed and able to work with minimal supervision.
- Demonstrates excellent observation, problem solving, verbal and written communication skills.
- Shows ability to organize and prioritize workload independently.
- Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
- Must be able to carry equipment weighing up to 10lbs. up and down stairs.
