Ambitious Minds Therapy Center & Primary Care is an established healthcare organization headquartered in Las Vegas, Nevada. As part of our continued commitment to expanding access to quality healthcare, we are excited to extend our services into the State of Utah, bringing compassionate, patient-centered primary care to individuals and families through innovative telehealth and mobile healthcare solutions.
We are seeking a compassionate, motivated, and patient-centered Family Nurse Practitioner (FNP) licensed in the State of Utah to become one of the founding providers of our Utah Primary Care team. This is an exciting opportunity to help shape and grow a new service area while being supported by an experienced leadership team. New graduates and newly licensed Nurse Practitioners are encouraged to apply!
Position Summary
The Family Nurse Practitioner will provide comprehensive primary care services primarily through telehealth while supporting patients enrolled in Utah Medicaid and Medicare programs. The provider may also perform in-home visits or travel to patient locations when clinically appropriate.

Responsibilities
- Provide comprehensive primary care services to patients across the lifespan.
- Conduct telehealth visits in compliance with Utah state regulations.
- Diagnose, treat, and manage acute and chronic medical conditions.
- Order, review, and interpret diagnostic tests and laboratory results.
- Prescribe medications and develop individualized treatment plans.
- Educate patients on disease prevention, wellness, and healthy lifestyle choices.
- Coordinate care with specialists, behavioral health providers, home health agencies, and community resources.
- Maintain accurate and timely electronic medical records (EMR).
- Participate in quality improvement initiatives and clinical team meetings.
- Provide in-home medical visits when needed.
- Travel within assigned service areas when necessary.
Qualifications Required
- Active Family Nurse Practitioner (FNP) license in the State of Utah.
- Active DEA registration (or ability to obtain).
- Current BLS Certification.
- Eligible to provide services for Utah Medicaid and Medicare patients.
- Strong communication and clinical decision-making skills.
- Reliable transportation and willingness to travel for patient care when necessary.
- Ability to provide mobile or in-home healthcare services as needed.
Preferred
- Experience in Primary Care, Family Medicine, Internal Medicine, Geriatrics, or Community Health.
- Telehealth experience (preferred but not required).
- Bilingual applicants are encouraged to apply.
