Cherish Home Health Network & Vcare Housecall Pro are seeking a dedicated and compassionate Nurse Practitioner (NP) to join our expanding doctor housecall and home‑based primary care team across the Dallas–Fort Worth metroplex.
Position Overview
The NP will provide in‑home clinical assessments, manage chronic conditions, coordinate care with PCPs and specialists, and support safe transitions from hospital to home. This role is ideal for clinicians who value autonomy, relationship‑based care, and culturally competent service.
Key Responsibilities
- Conduct comprehensive in‑home evaluations (acute, chronic, post‑hospital).
- Manage treatment plans, medications, and follow‑up care.
- Collaborate with PCPs, case managers, and home health teams.
- Complete documentation in EMR within required timeframes.
- Provide culturally sensitive care to Vietnamese‑speaking patients.
- Participate in quality improvement and care coordination workflows.
Qualifications
- Licensed Nurse Practitioner (NP) in Texas.
- Active DEA (or willingness to obtain).
- Experience in primary care, geriatrics, home health, or housecall medicine preferred.
- Bilingual English–Vietnamese-Spanish strongly preferred (Not Required)
- Strong clinical judgment and ability to work independently.
- Reliable transportation for home visits.

Pay: $110,004.55 - $132,478.61 per year
