We are seeking an empathetic, patient, compassionate, and organized individual to provide social services and support to older adults with physical and/or mental health disabilities in an adult day health care setting.
Requirements
- Spanish-speaking required
- Master’s degree in Social Work (MSW) preferred/required, or currently enrolled in an MSW program
- Bachelor’s degree in Social Work may be considered based on experience and program requirements
- Strong communication, organizational, and time-management skills
- Compassionate and patient approach when working with older adults and individuals with disabilities
- Ability to work collaboratively with clients and an interdisciplinary care team
Key Responsibilities
- Conduct biopsychosocial assessments to identify clients’ social, emotional, behavioral, and environmental needs.
- Screen for mental health concerns and monitor clients’ overall psychosocial well-being.
- Complete six-month reassessments to evaluate changes in clients’ mental health, functioning, and psychosocial needs.
- Collaborate with clients and the interdisciplinary team to develop individualized care coordination plans and review/update plans every six months to monitor progress.
- Provide case management services, including assisting clients with understanding and reviewing correspondence and documents from public social service agencies such as Medi-Cal and Social Security.
- Assist clients in accessing appropriate community resources, benefits, and social services.
- Facilitate psychoeducational and supportive groups on topics such as depression, anxiety, coping skills, and maintaining emotional well-being.
- Educate and empower clients on how to navigate public social service programs and better understand and manage their available benefits.
- Advocate for clients and assist them in identifying resources that promote independence, stability, and quality of life.
- Maintain accurate and timely clinical documentation in accordance with program and regulatory requirements.
- Collaborate with nurses, therapists, activity staff, physicians, and other members of the interdisciplinary team to support client-centered care.
