RN Field Supervisor
Queens/Nassau County Area
Earn $50-$55/hr.
The RN Field Supervisor is responsible for providing direct and indirect patient care services under a physician’s plan of care and in accordance with the policies and procedures of the state’s professional standards, and regulatory entities.
The candidate we seek will be responsible for:
- Receive and process incoming requests for new home care services
- Provide assessments of newly referred patients/clients, assess health needs and eligibility for homecare.
- Perform assessments to assigned patients/clients according to medical orders & policies and procedures in accordance with the RN scope of practice.
- Provide post hospitalization and ongoing re-assessments.
- Communicate with the team and physicians and obtain any medical orders as needed
- Develop and update the patient's care plan.
- Review the plan of care aide/s to ensure competency, as well as complete the annual aide in home supervisions.
- Ongoing training of aide/s at patients’ homes to ensure excellent patient care.
- Conduct weekly pre-employment skills assessment training for potential PCAs/HHAs and in- service education as needed.
- Maintain and manage patient/client files and maintain accurate and timely records of patient care.
- Provide patient/client and family discharge planning on admission and on an ongoing basis.
- Ensure compliance with all regulatory, as well as internal policies and procedures
- Participate in on-call rotation to support on-call team when clinical issues arise.
- Assist DPS with the supervision of all LPN’s/RN’s, in the nursing department.
- Other assignments that fall within the RN Practice Act.
- Perform any additional duties as assigned by management.
What we require from you…
- Bachelor’s degree with one year Nursing experience or Associate degree with two years Home Care experience as a Field RN.
- Excellent organizational & time-management skills.
- Basic computer skills; knowledge of email.
- Must have interpersonal relationship skills
- Ability to understand directions, carry out instructions and maintain records.
- Ability to assume delegated authority and accountability for care of assigned patients.
- Ability to function independently with minimal supervision.
- Must have supervisory experience and homecare experience.
- Meets applicable health requirements to provide patient care.
- If a state standard exists that is higher than these education/experience requirements, the state standard supersedes these qualifications.
- Demonstrated multi-tasking ability and effective organizational skills to meet short deadlines with finite resources.
- Demonstrated ability to develop, retain and manage an effective caregiver team.
- Able to engage in local and occasional out-of- town travel.
- Computer proficiency including the ability to utilize software programs for creating documents and data analyses.
- Applicable federal and state health requirements to provide client services as appropriate
Working Conditions & Physical Effort:
- Work is normally performed in a typical interior/office work environment as well as client residences and senior living facilities.
- Ability to work flexible schedule including overtime and evening hours as needed.
- Work may involve frequent exposure to communicable diseases, bloodborne pathogens, and/or other potentially infectious or hazardous materials that require following safety precautions and using work practices that require the use of personal protective equipment.
- Able to travel from assignment to assignment within the geographic area served.
- Medium physical activity that may require constant lifting, carrying, pushing, or pulling over 50lbs, & physically stretching, bending, stooping, twisting, sitting, walking, reaching, & climbing stairs.
- Able to frequently lift, reposition, and transfer clients and assist with standing and walking.
- Able to consistently stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs, & reach freely.
- Able to grasp with thump is opposition of fingers of palm (i.e., manipulate pen, knobs, or objects).
- Ability to visually and auditorily observe the patient/client.
