Job Summary
We are seeking a compassionate and dependable PRN Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. The LPN will work under the supervision of a Registered Nurse (RN) and in collaboration with the interdisciplinary care team to ensure high-quality, patient-centered care.
Responsibilities
- Provide direct nursing care to patients in their homes according to the plan of care
- Perform skilled nursing tasks such as:
- Medication administration
- Wound care and dressing changes
- Vital signs monitoring and patient assessments
- Foley catheter care and other treatments as ordered
- Observe, record, and report patient condition and any changes to the supervising RN
- Educate patients and caregivers on disease processes, medications, and home care needs
- Maintain accurate, timely, and complete clinical documentation in compliance with agency policy and regulations
- Follow infection control, safety, and HIPAA guidelines at all times
- Communicate effectively with patients, families, physicians, and agency staff
- Participate in case conferences or care coordination as needed
Qualifications
- Current and active LPN license in good standing in the state of practice
- Minimum 1 year of nursing experience preferred (home health experience a plus)
- Current CPR certification
- Valid driver’s license with reliable transportation and current auto insurance
- Ability to work independently and manage time effectively
- Strong communication and interpersonal skills
- Basic computer skills for electronic documentation
Physical Requirements
- Ability to lift, reposition, or assist patients as needed
- Ability to stand, walk, bend, and kneel during patient care activities
- Ability to travel to patient homes
