Nurses At Home is looking for an enthusiastic and compassionate Full-Time Physical Therapist to help expand our growing home health presence in Waterbury and the surrounding communities.

As our Full-Time Home Health Physical Therapist, you will provide skilled physical therapy services to patients in their homes while working closely with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team.
You will have the opportunity to help patients regain their strength, mobility, confidence, and independence following hospitalization, illness, injury, or a decline in function.
As our Waterbury territory grows, this position may also offer opportunities to assist with clinical program development, therapist mentoring, interdisciplinary communication, and future leadership responsibilities.
Complete comprehensive physical therapy evaluations and reassessments
Develop individualized plans of care based on each patient’s needs and goals
Provide skilled treatment focused on mobility, strength, balance, transfers, safety, and functional independence
Educate patients, families, and caregivers on exercises, equipment, and home safety
Coordinate care with nursing, occupational therapy, physicians, and other providers
Participate in interdisciplinary communication and care planning
Complete accurate and timely clinical documentation
Identify changes in condition and communicate concerns promptly
Help patients remain safely and successfully in their homes
Current Connecticut Physical Therapist license
Graduate of an accredited physical therapy program
Valid driver’s license and reliable transportation
Strong communication, organization, and documentation skills
Ability to work independently while remaining engaged with the clinical team
A positive attitude and genuine commitment to patient-centered care
Home health experience is preferred, but we are open to speaking with motivated therapists who have strong clinical experience and are excited to grow in the home health setting.
