Job Description
We are seeking a Full-Time Physician (MD/DO) to serve as a dedicated clinical leader across our patient population. This is a Monday–Friday role combining telehealth and in-person care, designed for a physician who values efficiency, autonomy, and high-impact patient care. We are looking for a consistent, engaged physician who takes ownership of patient outcomes and supports our advanced practice providers. We have 20 providers in total with several part time physicians and a Chief Medical Officer
Schedule & Structure
- 4 days/week: Telehealth visits (primary focus)
- 1 day/week: In-person visits (homes and assisted living facilities)
- 10–20 visits per day (blended, primarily telehealth)
Core Responsibilities
- Conduct new patient visits and transitional care management (TCM) visits via telehealth
- Perform in-person visits 1 day per week for:
- Annual or periodic check-ins (1–3x/year per patient)
- Quarterly visits for assisted living patients
- Serve as clinical escalation point for Nurse Practitioners and Physician Assistants
- Provide medical decision-making support for complex cases
- Ensure quality, compliance, and continuity across the care model
- Collaborate with interdisciplinary teams, including primary care, palliative care, and facility staff
- Document care using eClinicalWorks (eCW)
Ideal Candidate Profile
- MD or DO, board-certified/board-eligible
- Active Arizona medical license
- Experience in primary care, geriatrics, palliative care, or home-based care preferred
- Comfortable with high-volume telehealth workflows
- Strong decision-making and leadership skills
- Ability to work independently while supporting a team-based care model
Compensation & Structure
- Revenue share model
- Expected annual earnings: $220,000 – $480,000+ (based on productivity)
- Flexible engagement options:
- W-2 (benefits eligible)
- 1099 (independent contractor)
