Job Title: Long-Term Care Consultant Pharmacist
Location: South Shore, MA / Rhode Island

Job Summary
Seeking a Consultant Pharmacist who is a knowledgeable, experienced, detail-oriented, highly motivated self-starter to provide expert pharmaceutical care to residents in long-term care facilities. The Consultant Pharmacist will ensure optimal medication therapy outcomes by conducting comprehensive medication regimen reviews, ensuring regulatory compliance, and working closely with facility staff to promote safe and effective drug use.
Key Responsibilities:
- Perform monthly medication regimen reviews for LTC residents and document clinical findings in accordance with federal and state regulations
- Identify and address medication therapy problems including polypharmacy, drug interactions, contraindications and inappropriate medication use
- Provide clinical recommendations to prescribers and facility staff to optimize patient outcomes
- Collaborate with nursing staff, medical staff, and other healthcare professionals to ensure safe and effective medication use
- Monitor for adverse drug reactions and provide follow-up actions and documentation
- Assist facilities in maintaining regulatory compliance with federal and state-specific guidelines
- Provide in-service education and training to facility staff
- Participate in quality improvement initiatives related to medication use in the facility
- Maintain accurate and timely documentation of all activities using appropriate software systems
Qualifications:
- PharmD or BS in Pharmacy from an accredited school of pharmacy
- Current, active pharmacist license in the state(s) where services will be provided
- Prior experience in long-term care/geriatrics is preferred
- Strong knowledge of geriatric pharmacotherapy, medication management standards, and regulatory requirements
- Excellent communication, documentation, and interpersonal skills
- Ability to work independently, manage time effectively, and travel to assigned facilities
Preferred Qualifications:
- Certification in Geriatric Pharmacy (BCGP)
- Membership in LTC or geriatric specific organizations (e.g., ASCP)
- Experience providing education and training to healthcare staff
Work Environment:
- Travel to multiple long-term care or skilled nursing facilities
- May involve occasional evening or weekend work, based on facility needs
- Work may be conducted on-site or remotely as applicable
