White Crane Wellness Center is an award winning and innovative non-profit organization whose objectives are to empower older adults to improve and sustain their health and to serve as an innovative model for a new approach to healthy aging. Our organization's goal is to equip older adults with the requisite tools, knowledge, and support necessary to remain healthy, active, and engaged in the community. We are dedicated to preventative medicine, positive approaches toward aging, holistic health, and empowering people to take an active role in their own health care.
Position: Community Outreach Nurse (Part-time)
Location: Northwest Chicago communities
Requirements: Must be licensed as a Registered Nurse (RN) by the State of Illinois.
- Under the direction of the Executive Director, the Nurse coordinates and provides holistic health nursing services, including but not limited to: health education workshops; health screenings; one-on-one consultations and referrals.
- Develops effective partnerships with health outreach team and site managers.
- Maintains attendance records, monthly summaries, time sheets, and other information relative to project implementation.
- Seeks feedback about program effectiveness from staff and participants.
- Performs other duties as needed.
