JOB SUMMARY
The Dental Care Coordinator serves as the primary link between outreach, education, pop-up dental services, and connection to primary care services. This position plays a central role in planning, coordinating, promoting, and supporting community-based dental pop-up clinics designed to increase access to oral health services for rural and underserved populations.
The Dental Care Coordinator functions as both an outreach specialist and enabling services team member, ensuring patients successfully access dental care, remain engaged in ongoing treatment, and are connected to comprehensive medical, dental, and community resources when needed. The position supports continuity of care through patient navigation, appointment management, referrals, education, follow-up activities, and community partnership development.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Coordinate pop-up dental clinic schedules and logistics with schools, childcare centers, senior housing facilities, community centers, and other community partners.
- Develop and maintain positive working relationships with community organizations and service sites.
- Promote awareness of available dental and primary care services through outreach and education activities.
- Represent the health center at community meetings, events, and outreach activities.
- Assist with recruitment and engagement of patients for pop-up clinic services.
- Serve as the primary point of contact for patients participating in pop-up dental services.
- Conduct direct outreach to patients, parents, caregivers, and families regarding available services.
- Schedule appointments and manage clinic rosters to maximize utilization of available appointment slots.
- Perform appointment reminders and confirmations to reduce no-show rates.
- Assist patients with registration, eligibility screening, and sliding fee discount enrollment.
- Facilitate referrals to fixed-site dental clinics, specialty providers, primary care providers, and other community resources.
- Coordinate follow-up care and ensure treatment recommendations are completed.
- Provide warm handoffs to medical, dental, behavioral health, pharmacy, and enabling service programs as appropriate.

EDUCATION
High School Diploma or GED required.
EXPERIENCE
Two years in dental, social services or clinical setting preferred.
