Physical Therapist (PT) – PRN

We are seeking a PRN Physical Therapist (PT) to support our rehabilitation team. This role offers flexible scheduling while providing the opportunity to help residents maximize mobility, strength, and independence in a skilled nursing and post-acute care setting.
Position Responsibilities:

Complete physical therapy evaluations and develop individualized treatment plans

Provide treatment focused on gait, balance, strengthening, transfers, mobility, endurance, and pain management

Monitor resident progress and modify treatment plans as appropriate

Collaborate with PTAs, OTs, SLPs, nursing staff, and physicians to coordinate resident care
-
Educate residents and caregivers on exercises and safety techniques
-
Complete accurate and timely documentation

Qualifications:

Active New Hampshire Physical Therapist license required

Strong communication and clinical reasoning skills

Skilled nursing experience preferred but not required
