Responsibilities
- Provide attentive patient observation to monitor health status and report changes promptly
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Administer medication as prescribed and document medication administration accurately in electronic health record (EHR) systems
- Support patients with medical procedures such as catheter care, vital signs monitoring, and basic life support (BLS) or CPR when necessary
- Offer specialized care for individuals with dementia, Alzheimer’s disease, or other cognitive impairments, including memory care strategies
- Assist with end-of-life and hospice care, ensuring comfort and dignity for patients in their final stages of life
- Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment
Qualifications
- Proven experience working with individuals with developmental disabilities or in assisted living environments
- Knowledge of medical terminology and home & community care regulations
- Prior experience in geriatrics, long-term care, or nursing home settings is highly desirable
- Certification in first aid and CPR required; additional certifications such as Hoyer lift training or hospice care are a plus
- Ability to accurately document medical records and use EMR/EHR systems efficiently
- Strong communication skills to collaborate effectively with healthcare teams and family members
- Physical ability to perform heavy lifting, assist with patient transfers using Hoyer lifts or similar equipment, and support mobility needs
