Overview
We are seeking a dedicated and experienced Travel Registered Nurse (RN) specializing in Case Management to join our healthcare team on day shifts, working 36 hours per week. This role offers an exciting opportunity to coordinate and oversee patient care across various clinical settings, ensuring seamless transitions and optimal health outcomes. The ideal candidate will possess extensive clinical expertise, strong communication skills, and a comprehensive understanding of healthcare regulations. Your primary responsibility will be managing patient cases efficiently while collaborating with multidisciplinary teams to deliver exceptional patient service.
**Must have compact RN license. They must also be willing to apply for Oregon and D.C. licensure within 48 hours of offer or offer will be revoked, no exceptions. **
Duties
- Conduct comprehensive patient assessments, including vital signs, medical history, and physiological evaluations to determine care needs.
- Develop and implement individualized care plans, coordinating discharge planning and post-acute care services.
- Manage utilization review processes, ensuring appropriate resource use in compliance with healthcare policies and managed care standards.
- Collaborate closely with healthcare professionals across hospital departments, outpatient clinics, and home health services to facilitate continuity of care.
- Maintain accurate electronic health records (EHR) and medical records, ensuring compliance with HIPAA regulations.
- Perform diagnostic evaluations and oversee case management activities for diverse patient populations including pediatrics, geriatrics, behavioral health, and complex acute cases.
- Engage in patient education initiatives, providing clear communication about treatment plans and health management strategies.
- Monitor patient progress through ongoing assessments, adjusting care plans as necessary to meet evolving needs.
- Coordinate with insurance providers for authorization and documentation related to hospital medicine, hospice care, or outpatient services.
- Ensure compliance with all relevant health regulation policies while maintaining high standards of patient service.
Skills
- Extensive critical care experience including ICU, emergency room, trauma centers (Level I & II), and infectious disease care.
- Proven background in hospital medicine, medical-surgical units, outpatient clinics, or post-acute settings such as home health or hospice.
- Strong knowledge of physiology, medical terminology, diagnostic evaluation techniques, and clinical case management principles.
- Experience working within EMR/EHR systems with proficiency in electronic health records management.
- Familiarity with HIPAA regulations and healthcare privacy standards.
- Excellent patient assessment skills combined with the ability to manage complex cases efficiently.
- Effective client communication skills to engage patients and collaborate with multidisciplinary teams.
- Knowledge of health regulation policies related to managed care, discharge planning, utilization management, and inpatient/outpatient services.
- Ability to adapt quickly in high-pressure environments such as emergency medicine or trauma centers.
- Demonstrated commitment to patient service excellence and ethical nursing practices.
