Wound Care Nurse Practitioner
Overview
We are seeking a highly skilled and compassionate Wound Care Nurse Practitioner to join our multidisciplinary healthcare team. This advanced practice provider will deliver specialized wound management, collaborate with physicians and other healthcare professionals, and contribute to patient recovery across diverse settings. The ideal candidate will possess extensive clinical expertise in wound healing, infection control, and complex patient assessments, ensuring optimal outcomes for patients with acute and chronic wounds. Experience with EMR/EHR systems such as Athenahealth, Epic, and Cerner. is preferred, along with a strong foundation in medical documentation, wound dressing, and patient education.
Responsibilities
- Conduct comprehensive wound assessments, including evaluating wound etiology, size, depth, and signs of infection.
- Develop individualized wound care plans incorporating advanced treatments such as PRP, debridement, wound dressing and biological skin substitute application when appropriate.
- Administer injections and medications following HIPAA regulations and DEA guidelines.
- Manage complex cases involving diabetic foot ulcers, pressure injuries, vascular wounds, and infectious disease-related wounds.
- Collaborate closely with physicians, physical therapists, dietitians, and other healthcare professionals to coordinate comprehensive patient care.
- Document all patient interactions accurately within Athenahealth while maintaining compliance with HIPAA standards.
- Provide patient education on wound prevention strategies, lifestyle modifications, and post-treatment care plans.
- Participate in clinical research initiatives to advance wound care practices and improve treatment protocols.
- Assist in discharge planning/post op follow up for inpatient cases and facilitate transitions of care across settings such as home health agencies.
Requirements
- Current Nurse Practitioner licensure with certification in wound care or related specialty preferred.
- Experience in emergency medicine or hospital medicine is highly desirable.
- Proficiency with EMR/EHR systems including Epic, Cerner and Athenahealth.
- Familiarity with coding systems including CPT, ICD-9/10 for accurate documentation.
- Strong understanding of physiology, anatomy knowledge related to skin integrity and healing processes.
- Ability to perform comprehensive physical examinations and diagnostic evaluations effectively.
- Excellent client communication skills with experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory/dementia care), or individuals with disabilities.

Pay: $120,000.00 - $135,000.00 per year
