Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong background in patient care and demonstrate the ability to work effectively in various medical environments, including hospitals, nursing homes, and assisted living facilities. This role requires excellent clinical skills, a commitment to patient advocacy, and the ability to collaborate with multidisciplinary teams to provide high-quality care.
Duties
- Provide direct patient care under the supervision of registered nurses and physicians.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Conduct patient assessments, including vital signs monitoring and health history intake.
- Assist with diagnostic evaluations and medical procedures, including IV insertion and catheter care.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Collaborate with healthcare professionals to develop and implement individualized care plans.
- Educate patients and families on health management, medication administration, and post-care instructions.
- Support patients with activities of daily living (ADLs) while promoting independence.
- Participate in infection control measures and ensure a safe environment for patients.
Experience
Candidates should have experience or familiarity with the following areas:
- PCC or similar electronic health record systems.
- Long-term care, assisted living, or nursing home environments.
- Experience working with individuals with developmental disabilities or dementia care.
- Knowledge of acute pain management techniques and hospice care practices.
- Proficiency in medical terminology, ICD coding (ICD-9/ICD-10), and medical documentation processes.
- Skills in phlebotomy, specimen collection/processing, and administering injections.
- Familiarity with geriatric care principles and senior health issues.
