Pay Rate: $76,693.00 per year with room for advancement based on years of service with organization. Additional compensation for emergency shifts and worked holidays.
Shift: Monday - Friday, 8 AM - 4:30 PM
Position Summary
The Float Nurse provides flexible clinical support to the hospice team throughout the CareFirstNY service area (Chemung, Schuyler, and Steuben counties) by covering patient caseloads, conducting visits based on patient and family needs, performing personal care visits, completing admissions, and carrying out additional nursing responsibilities as assigned by the Leadership Team during standard business hours, Monday – Friday, 8AM – 4:30 PM. Occasional changes in schedule may be necessary for emergent operational or patient care needs.
Essential Duties and Responsibilities
- Share in on-call rotation providing 24-hour, 7-days-a-week coverage to patients and families
- Provide information visits and admissions, as assigned
- Provide coverage for Case Managers as needed including completing assessments of patient/family needs, development of care plans, communication/education in a manner to advocate and empower, and complete nursing in fiscally responsible manner
- Provide end-of-shift report via phone call to oncoming shift nurse when providing on-call coverage per agency need
- Participate in interdisciplinary group meetings to update patient/family plan of care as needed
- Carry a caseload assigned by Team Manager(s) per agency need
- Participate in quality assurance program and perform chart audits as assigned
- Consult with Team Manager(s) or designee, and/or other interdisciplinary team members before acting on changes in patient’s health status, community referral potential, family stresses, and unanticipated nursing or medical needs
- Work with and collaborate with all members of the interdisciplinary team as indicated
- Maintain timely documentation according to agency guidelines
- Provide supervision and evaluation of hospice and contracted homemakers, home health aides, and LPN’s
- Participate in staff development
- Other duties as assigned
Qualifications
Qualified candidates will possess a valid and current NYS Professional Registered Nurse’s license and driver’s license with a minimum of 2 years’ experience in home care and/or acute care setting as a professional nurse within the last 10 years. Must possess physical assessment skills, strong communication skills, and be skilled in the use of nursing processes.
Additional preferred experience(s)/education:
- Know and understand state and federal regulations relating to hospice and palliative care
- Knowledge of Medicare, Medicaid, and private insurance guidelines
