Post Acute Specialists of Florida is hiring Telehealth Care Coordinators who are passionate about improving access to high-quality healthcare for seniors. Our team expands post-acute care services through innovative telehealth solutions, delivering specialized care directly to seniors wherever they live.
- This is a driving position that serves as an on-site support partner for virtual medical visits. You will regularly travel to post-acute care facilities and private residences throughout the Tallahassee, FL area to be present during telehealth sessions, ensuring seniors are prepared, comfortable, and ready for their visits. In this role, you help bridge the gap between patients and remote healthcare providers.
- This position is part-time and as-needed to start, with work scheduled within the company’s service hours of 7:00 AM to 5:00 PM, Monday through Friday. Availability during these hours is required. Based on performance and business needs, this role may lead to full-time opportunities in the future.
Supporting Seniors Through Telehealth. As a Telehealth Care Coordinator, you will provide hands-on, in-person support to prepare patients for virtual medical appointments. Responsibilities include reassuring patients, answering questions, and helping them feel confident using telehealth technology. You may work in hospitals, skilled nursing facilities, rehabilitation centers, or patients’ homes, adapting your approach to meet each individual’s needs.
Technology Assistance & Appointment Support. You will assist patients with setting up and using telehealth equipment, internet connectivity, and video platforms. Your calm, patient approach will help ensure seniors can participate in virtual visits comfortably and without unnecessary stress.
Skills for Success. Successful candidates are eager to learn new technology and adapt to evolving processes. With training provided, you will develop the skills needed to troubleshoot basic technical issues and support patients throughout their telehealth experience. A flexible mindset, strong communication skills, and a service-oriented attitude are essential.
Qualifications.
- Strong interpersonal skills with a patient, empathetic approach
- Ability to quickly learn new technology and follow updated procedures
- Experience working with seniors in post-acute care environments such as hospitals, rehabilitation centers, or skilled nursing facilities
- Familiarity with workflows and patient care practices in SNFs or similar settings
- Basic understanding of medical terminology, healthcare operations, HIPAA regulations, and electronic health systems
- Required Credentials & Driving Requirements
- Active CNA or MA certification with a minimum of 2 years of recent experience
- PCT or Home Health experience is a plus
- Current BLS certification
- Proficiency in measuring and documenting vital signs
- TB screening completed within the past 6 months
- High School Diploma or GED
- Clear criminal background check
- Valid driver’s license with a clean motor vehicle record (MVR)
- Use of reliable personal vehicle with current auto insurance
