RN Diabetic Nurse for Home Health
Location: Salt Lake County (ALFs and Patient's Homes)
Pay: $25 per Visit per Patient, $10/hour for drive time
Schedule: 7am-3pm, 7 days on then 7 days off (Wednesday - Tuesday rotation)
Join our growing team providing compassionate diabetic care to patients throughout Salt Lake County. This full-time role offers a consistent schedule, meaningful patient relationships, and the opportunity to work independently in the field.
Schedule:
- Full-time, Wednesday through Tuesday (7 on/7off)
- 7:00 AM – 3:00 PM
What You’ll Do:
- Perform insulin administration and weekly pen fills
- Provide diabetic care in assisted living facilities and patient homes
- Manage up to 21 patient visits per day
- Monitor patient health and maintain accurate documentation
What We Offer:

- Assigned coverage area in either North or South Salt Lake County
- Most visits located within assisted living facilities and patients' homes
- Paid drive time ($10/hour) plus gas reimbursement when traveling to patient homes
What We’re Looking For:
- Active Utah nursing license
- Strong knowledge of diabetes management
- Compassionate, dependable, and organized
- Ability to work independently and efficiently
