POSITION OVERVIEW
The Registered Nurse (RN) at Care Indeed provides clinical leadership in the delivery of skilled, compassionate nursing care across home and community-based settings. RNs oversee and guide each client’s plan of care, integrating clinical expertise with patient advocacy to drive optimal outcomes.
This role is centered on comprehensive assessment, coordinated care planning, education, and interdisciplinary collaboration with the care team, physicians, clients, and families. RNs are expected to exercise sound clinical judgment, communicate with clarity and professionalism, and model best practices in every interaction.

DUTIES & RESPONSIBILITIES
Conduct comprehensive health and psychosocial assessments, including Start of Care, ongoing reassessments, and recertification visits
Develop, implement, and revise the patient plan of care based on clinical findings, patient goals, and physician orders
Administer medications and treatments as ordered and perform medication reconciliation to ensure accuracy and safety
Manage wound assessment and wound care in accordance with clinical standards and orders
Communicate directly with physicians and care partners to obtain, clarify, and update orders and care plans
Lead care coordination and provide clinical supervision and delegation to LVNs, HHAs, and HCAs in the field
Provide patient and caregiver education that supports safety, self-care, rehabilitation, and care plan adherence
Observe, assess, document, and report patient condition, symptoms, responses to treatment, safety concerns, and changes using clear clinical terminology
Complete timely, accurate documentation, including clinical notes, progress notes, and care coordination records, using EMR systems such as WellSky via mobile or tablet platforms
Participate in team conferences and in-service learning that strengthen clinical practice and collaboration
JOB CONDITIONS
Provide care across varied settings, including private homes, assisted living communities, and care facilities
Travel between client locations using reliable personal or public transportation; a valid driver’s license and current auto insurance are required when driving
Safely navigate and access client living environments with differing layouts and accessibility
Perform physical nursing duties, including walking, standing, bending, reaching, lifting, turning, and repositioning patients (up to 50 lbs)
Maintain visual acuity, hearing, and manual dexterity needed for assessments and skilled nursing care
Work in environments with potential exposure to bloodborne pathogens and infectious materials
QUALIFICATIONS
Active, unrestricted Registered Nurse (RN) license in the State of California, maintained throughout employment
Graduation from an accredited school of nursing
Minimum one (1) year of verifiable experience (2,000 hours) within the last three (3) years as an RN in a licensed home health agency
Current Cardiopulmonary Resuscitation (CPR) certification issued by the American Heart Association, maintained throughout employment (may be obtained within two weeks of hire)
Working knowledge of home health regulations, standards of practice, and required clinical documentation
Demonstrated ability to perform comprehensive assessments, develop and revise care plans, and coordinate care across the clinical team
Experience with electronic medical record systems, such as WellSky, using mobile or tablet-based documentation
Clear spoken and written English for documentation and care coordination, with the ability to communicate with clients in the language that supports their comfort and understanding whenever possible
Successful completion of Live Scan fingerprint background clearance prior to employment (in-house option available)
Ability to provide required professional references, pass medical clearance, and complete background screening as part of the hiring process
CRP PROVISION (Two-Week Window Option)
A valid American Heart Association CPR certification is required. Candidates may be hired before obtaining it, but must secure certification within two weeks of hire and maintain it throughout employment. Proof must be submitted to the supervisor within the deadline.
LICENSURE/CERTIFICATION MAINTENANCE
Required licensure/certification/registration must be maintained throughout employment. Proof of current, valid credentials must be provided upon request.
WORK ENVIRONMENT & CARE SETTINGS
Care is provided in diverse settings, including private homes, assisted or independent living communities, skilled nursing facilities, and hospitals, and involves close, direct interaction with clients. This role may include exposure to blood, body fluids, or other potentially infectious materials. It may require responding appropriately to urgent or emergencies in accordance with training and agency protocols. Some assignments may require local travel.
WORK SCHEDULE
Work schedules vary based on client needs and may include short or extended shifts, as well as day, evening, weekend, or holiday assignments.
