Manhattan Home Care Ltd is a Licensed Home Care Services Agency (LHCSA) committed to delivering high-quality, compassionate care to individuals throughout New York City and Long Island. We are currently seeking experienced and motivated Registered Nurses to join our growing team as UAS Assessors.
Position Summary
The UAS Assessor Registered Nurse is responsible for conducting face-to-face Uniform Assessment System (UAS-NY) evaluations for individuals seeking home care services. The RN will perform comprehensive clinical assessments, document findings accurately, determine eligibility for services, and collaborate with enrollment and care management teams to support appropriate care planning and service authorization.
Responsibilities
- Conduct in-home and community-based UAS-NY assessments.
- Evaluate members' medical, functional, cognitive, behavioral, and social needs.
- Complete and submit accurate assessment documentation within required timeframes.
- Develop initial care plans based on assessment findings.
- Educate prospective members and families regarding MLTC plan benefits, services, and member rights.
- Explain plan features, enrollment requirements, and member handbook information.
- Communicate assessment outcomes and recommendations to Enrollment and Care Management departments.
- Maintain accurate electronic medical records and comply with all regulatory requirements.
- Manage assigned caseload and schedule efficiently while meeting productivity goals.
QualificationsRequired
- Active New York State Registered Nurse (RN) license in good standing.
- Minimum one (1) year of home care, community health, MLTC, or field assessment experience.
- Strong clinical assessment and documentation skills.
- Valid New York State driver's license.
- Reliable transportation and willingness to travel within assigned territories.
- Proficiency with electronic documentation systems and basic computer applications.
- Ability to work independently and manage a flexible schedule.
Preferred
- Bachelor's Degree in Nursing (BSN).
- UAS-NY certification and assessment experience.
- Experience with Managed Long-Term Care (MLTC), Medicaid, or home care services.
