About the Role:
The Rn Case Manager coordinates and facilitates safe, timely, and appropriate patient discharges by assessing needs, developing individualized discharge plans, and collaborating with patients, families, healthcare providers, community agencies, and post-acute care facilities. Arranges follow-up services, equipment, medications, transportation, and community resources while addressing barriers to discharge. Maintains accurate documentation, communicates effectively with interdisciplinary teams, and ensures continuity of care and compliance with organizational regulatory requirements.
Minimum Qualifications:

Current, valid , and unencumbered Registered Nurse (RN) license that allows practice int he State of Texas.

Bachelor of Science in Nursing (BSN) or equivalent nursing degree.

Minimum of four years of clinical nursing experience, preferably in case management or a related healthcare setting.

Strong knowledge of healthcare regulations, insurance processes, and patient care standards.

Excellent communication, organizational, and critical thinking skills.
Preferred Qualifications:

Certification in Case Management (CCM) or equivalent credential.

Experience working with diverse patient populations and complex medical conditions.

Familiarity with electronic health record (EHR) systems and case management software.

Familiarity with MCG criteria is preferred
Responsibilities:

Conduct comprehensive assessments of patients' medical, psychosocial, and environmental needs to develop personalized care plans.

Coordinate with physicians, nurses, social workers, and other healthcare professionals to implement and monitor care plans effectively.

Facilitate communication between patients, families, and healthcare providers to ensure understanding and adherence to treatment protocols.

Monitor patient progress and outcomes, making necessary adjustments to care plans to address changing health conditions.

Ensure documentation accuracy and compliance with healthcare regulations, insurance requirements, and organizational policies.

Provide education and support to patients and families regarding disease management, treatment options, and available community resources.

Participate in quality improvement initiatives to enhance case management processes and patient care delivery.
Skills:
The RN Case Manager utilizes clinical expertise and critical thinking skills daily to assess patient needs and develop effective care plans. Strong communication skills are essential for coordinating with multidisciplinary teams and educating patients and families about treatment options. Organizational skills enable the management of multiple cases simultaneously while ensuring compliance with documentation and regulatory standards. Proficiency with electronic health records and case management software supports efficient tracking of patient progress and facilitates data-driven decision-making. Additionally, problem-solving abilities help the RN Case Manager address barriers to care and optimize resource utilization to improve patient outcomes.
Hours: M-F 8:30 AM-5:00 PM.
This is a FT position budgeted at 40 hours per week. This position, if applicable is eligible for overtime oay.
