Responsibilities
- Observe and monitor patients’ conditions closely, documenting any changes accurately using electronic health record (EHR) systems while maintaining strict compliance with HIPAA regulations.
- Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, toileting, and mobility support using equipment such as Hoyer lifts when necessary.
- Take vital signs such as blood pressure, pulse, temperature, and respiration rates to help assess patient health status.
- Administer medications and treatments as prescribed by medical professionals while ensuring proper documentation and adherence to safety protocols.
- Support patients with specialized needs including dementia care, Alzheimer’s care, hospice care, and working with individuals with developmental disabilities or disabilities.
- Perform catheter care, wound care, and other medical procedures under supervision or according to training protocols.
- Maintain a clean and safe environment by assisting with laundry, cleaning resident areas, meal preparation, and ensuring infection control standards are met.
- Use EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems efficiently to document patient information accurately and promptly.
- Collaborate with nursing staff and other healthcare professionals to deliver comprehensive patient care tailored to individual needs in home & community settings or long-term care facilities.
- Engage in ongoing training related to medical terminology, CPR certification, dementia/dementia-specific memory care techniques, and hospice procedures to stay current with best practices.
Qualifications
- Valid CNA certification from an accredited program; prior experience in nursing homes or assisted living preferred but not required.
- Proven experience working with seniors or individuals with disabilities; familiarity with Alzheimer’s and dementia care is highly valued.
- Knowledge of medical terminology and basic clinical skills such as vital signs measurement and medication administration.
- Ability to operate EMR/EHR systems efficiently while maintaining confidentiality in accordance with HIPAA standards.
- Strong communication skills; compassionate approach to patient care; ability to work effectively both independently and as part of a team.
- Physical stamina to stand for extended periods; ability to lift or assist residents using appropriate equipment like Hoyer lifts when necessary.
- CPR certification required; additional certifications such as catheter care or hospice training are advantageous.
- Experience in home health or community-based caregiving environments is a plus; willingness to work flexible hours including live-in employment options is desirable.
