RN Care Manager

Position Summary
UniVida Medical Centers is seeking a compassionate and detail-oriented Care Manager to support patients with chronic and complex health conditions. The Care Manager will coordinate care across the healthcare continuum, helping patients access the services, resources, and support needed to improve health outcomes and quality of life.
Key Responsibilities
- Conduct patient assessments and develop individualized care plans
- Coordinate care with primary care providers, specialists, hospitals, and health plans
- Support high-risk patients, including those with chronic conditions, multiple comorbidities, medication adherence challenges, and recent hospitalizations
- Facilitate hospital follow-up and Transition of Care (TOC) activities
- Educate patients regarding medical conditions, treatment plans, preventive care, and available community resources
- Assist with HEDIS gap closure, preventive screenings, medication adherence, and risk stratification initiatives
- Coordinate services related to behavioral health, social services, and community resources
- Participate in interdisciplinary care team meetings and collaborate with clinical staff to improve patient outcomes
- Maintain accurate documentation in electronic health records and care management systems
Qualifications
- High School Diploma or equivalent required
- Minimum 2 years of healthcare-related experience preferred
- Certification or licensure as a Medical Assistant, LPN, RN, Foreign Medical Graduate, or equivalent clinical experience
- Strong communication, organizational, and problem-solving skills
- Experience with EHR systems and Microsoft Office applications
- Bilingual (English/Spanish) preferred
