Part-Time LPN – Hands On Assistance
Location: Plymouth, MN
Schedule: Part-time | Flexible shifts from 8:30am-1:30pm, 3:30pm-8:30pm, and/or 8:30am-8:30pm
About the Role:
Gianna Homes is looking for a kind, dependable Licensed Practical Nurse (LPN) who is passionate about providing personal direct care in a home like assisted living setting. You will support an adult with quadriplegia while assisting team members with six dementia care residents helping them live safely, comfortably, and with dignity. This position is perfect for an LPN who values connection, consistency, and making a real difference in someone’s everyday life.
What You’ll Do:
- Provide attentive, hands-on care in a private home like assisted living
- Assist with daily routines such as personal care, positioning, and mobility
- Transport/drive resident in resident owned vehicle as needed
- Monitor comfort, safety, and overall well-being
- Administer medications and provide basic nursing care
- Support bowel/bladder care, catheter care, ostomy care and skin integrity
- Communicate closely with the care team and RN supervisor
- Create a calm, respectful, and supportive environment
What We’re Looking For:
- Active LPN license in good standing
- A caring, patient, and reliable personality
- Comfort working one-on-one with a resident with physical limitations
- Valid Minnesota Driver’s License
