JOB SUMMARY
The Job Coach is responsible for on-site job training, mentoring, coaching, and workplace assessment services for individuals in the community. Locations needed: Lowell, Hebron and Jasper and Porter County.
DUTIES/RESPONSIBILITIES

Provide coaching and training to actively working Vocational Rehab clients.

Provide coaching and on-going training to clients receiving Extended Services through Waiver and Community Support budgets as outlined in their budgets.

Assist with the development of natural supports at the worksite.

Monitor and document client progress on struggles and successes to maintain established goals.

Maintain necessary documentation, data and reports as required by CARF (Commission of Accreditation Rehabilitation Facilities) standards, funding sources and agency.

Maintain necessary documentation daily, weekly, and monthly to adhere to department standards.

Advocate for and pursue services appropriate to meet the individual needs of clients.

Maintain the required productivity level at 85% client specific productivity hours weekly based on caseload of active job coaching and follow-along.

Develop and maintain productive relationships with local employers, referral sources, clients, parents, co-workers, and others as appropriate.

Become familiar with client specific information by attending client conferences and reading files prior to onset of coaching services, including but not limited to BSP (Behavior Support Plan) training.

Attend and participate in department, division, and agency meetings, and training required.

Maintain awareness of local, state, and national trends that impact employment services.

Demonstrate appropriate professionalism and be a role model for team members and clients.

Assist and Provide Discovery Services as needed and assigned.

Your caseload will likely require working in Porter, Lake, and surrounding counties as well as various shifts including nights and weekends.

EDUCATION AND EXPERIENCE

High School Diploma
-
1-3 years' experience in related field preferred
-
Experience with developmentally disabled population preferred
