The Stoughton Senior Center is seeking a compassionate, self‑motivated Senior Center Case Manager to support adults 55+ in maintaining independence and enhancing their quality of life. As a Senior Center Case Manager, you will interview clients and their support networks to identify needs and determine appropriate services. You will advocate for older adults, assist with housing options, coordinate in‑home support, and help clients navigate important applications related to financial assistance, food security, and healthcare. The role includes assessing home safety, monitoring services, reporting abuse or neglect when necessary, and maintaining accurate records. You’ll also provide outreach to the community, attend meetings and training, and stay informed about programs and resources that benefit older adults. Success in this role requires strong communication skills, sound judgment, excellent organization, and the ability to maintain confidentiality.
This position requires a bachelor’s degree in a human‑service field or comparable experience, knowledge of older adult resources, experience working with seniors, and proficiency with Microsoft Office and related software. Candidates must have or be able to obtain CPR certification within six months of hire, pass required pre‑employment checks, a physical and drug screen, and possess a valid Wisconsin driver’s license and reliable transportation. The role involves a variety of physical and environmental demands, including home visits that may expose the worker to pets, smoke, or challenging living conditions.
