We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team within a Skilled Nursing Facility (SNF) in Minnesota. In this role, you will work closely with residents recovering from illness, injury, surgery, or age-related conditions to improve mobility, restore function, and maximize independence.
This position is ideal for therapists who enjoy working with the geriatric population and collaborating with an interdisciplinary team focused on delivering exceptional patient-centered care.

Evaluate residents and develop individualized physical therapy treatment plans based on clinical findings and patient goals.

Provide skilled therapeutic interventions, including strength training, balance training, gait training, transfer training, and mobility exercises.

Assist residents in improving functional mobility, safety awareness, and activities of daily living.

Monitor patient progress and modify treatment plans as appropriate.

Collaborate with interdisciplinary care teams including Occupational Therapists, Speech-Language Pathologists, nursing staff, physicians, and case managers.

Educate residents, families, and caregivers regarding safety, fall prevention, mobility strategies, and home exercise programs.

Participate in care conferences, discharge planning, and quality improvement initiatives.

Complete timely and accurate documentation in accordance with facility, state, and federal regulations.

Graduate of a CAPTE-accredited Physical Therapy program.

Current Minnesota Physical Therapist License issued by the Minnesota Board of Physical Therapy, or eligibility to obtain licensure prior to start date.

Successful completion of the National Physical Therapy Examination (NPTE).

Current CPR/BLS certification or ability to obtain upon hire.

Skilled Nursing Facility, rehabilitation, or geriatric experience preferred but not required.

Strong communication, interpersonal, and documentation skills.

Ability to assist residents with transfers, ambulation, positioning, and mobility activities.

Frequent standing, walking, bending, reaching, and lifting throughout the workday.

Ability to lift, move, or support up to 50 pounds with assistance as needed.

Ability to safely perform patient handling activities in accordance with facility guidelines.
