Responsibilities
- Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary.
- Monitor patient health through vital signs assessment, medication administration, and detailed medical documentation while adhering to HIPAA regulations to ensure privacy and confidentiality.
- Observe and report on patient conditions, including changes in cognition or physical health, utilizing EMR (Electronic Medical Record) systems for accurate record keeping.
- Support clients with specialized needs such as dementia or Alzheimer’s care, memory care routines, hospice support, and working with individuals with developmental disabilities or disabilities.
- Prepare nutritious meals and assist with light housekeeping tasks like cleaning and laundry to promote a safe and comfortable living environment.
- Utilize medical terminology accurately when documenting patient information or communicating with healthcare teams.
- Operate medical equipment safely, including catheter care tools, Hoyer lifts, and other assistive devices to ensure patient safety during transfers and daily activities.
Qualifications
- Valid CNA certification or equivalent certification in caregiving; prior experience in assisted living, nursing homes, or home health settings is highly preferred.
- Demonstrated experience working with individuals with disabilities or special needs; familiarity with dementia or Alzheimer’s care is a plus.
- Knowledge of medical procedures such as vital signs measurement, medication administration, catheter care, and hospice support.
- Proficiency in using EMR/EHR systems for accurate documentation; understanding of medical terminology is advantageous.
- CPR certification required; additional certifications such as hospice care training or Hoyer lift operation are beneficial.
- Ability to work independently in a home setting while maintaining high standards of patient safety and confidentiality.
- Strong communication skills to effectively collaborate with clients’ families and healthcare professionals.
