Job Summary

Responsibilities:
- Conduct telephonic and occasional in-home assessments of members’ medical, psychosocial, physical, and spiritual needs.
- Develop, implement, and monitor Person-Centered Service Plans (PCSP) using UAS-NY data.
- Respond to changes in member condition or sentinel events by updating care plans and communicating with the interdisciplinary team.
- Collaborate with PCPs, specialists, member representatives, and internal care teams to ensure continuity of care.
- Educate members on disease processes, preventive health, and quality-of-life strategies.
- Participate in interdisciplinary care team meetings and present member cases.
- Utilize benefit structures and community resources to meet member needs.
- Ensure compliance with CMS and DOH guidelines.
Qualifications:
- Graduate of an accredited nursing program; BSN preferred.
- Current RN license in New York State.
- CCM certification preferred.
- 3–5 years of nursing experience in long-term care, managed care, home care, geriatrics, or hospice.
- Prior experience in interdisciplinary service delivery environments.
- Working knowledge of Medicare and Medicaid regulations.
- Strong communication, organizational, and care coordination skills.
