We are seeking Occupational Therapist/ Occupational Therapist Assistants to provide in-home therapy services to patients in Orange, CA. The ideal candidate will have a passion for helping others regain independence, excellent communication skills, and the ability to work independently while collaborating with an interdisciplinary team.
Responsibilities:
- Conduct initial evaluations and develop individualized care plans based on patient needs.
- Provide therapeutic interventions focused on improving daily living skills, mobility, and safety in the home environment.
- Educate patients and caregivers on home exercise programs, adaptive equipment, and strategies to enhance independence.
- Document all visits, progress notes, and care updates accurately and in compliance with home health regulations.
- Collaborate with nurses, and other disciplines to ensure coordinated and effective care.
- Maintain accurate documentation in compliance with agency and regulatory requirements.
- Monitor progress and adjust care plans as needed to achieve optimal outcomes.
Requirements/Qualifications:
- Occupational Therapist/Occupational Therapist Assistant License
- Active Driver's License
- CPR Certification
- Liability Insurance
- Auto Insurance
- Experience in Home Health (Preferred)
This is an independent contractor position (1099); CA business license preferred.
