We're looking for an experienced, self-driven RN to join our PRN team serving Cooke County patients in the comfort of their own homes.
What You'll Do
- Complete thorough initial and ongoing patient assessments to guide individualized care.
- Develop, initiate, and revise personalized care plans based on evolving patient needs.
- Administer skilled nursing services as ordered by the physician.
- Evaluate patient outcomes and continuously adjust your approach to deliver the best results.
- Coordinate with the broader care team to ensure seamless, well-managed service.
- Monitor cases for compliance with third-party payor requirements.
- Maintain accurate, timely clinical and progress notes.
What You Bring
- Active Texas RN license or multi-state compact license.
- 1+ years of RN experience — home health, hospice, or community nursing strongly preferred.
- Current CPR certification and valid Texas driver's license.
- Reliable transportation and valid auto liability insurance.
- Strong interpersonal skills and the ability to work independently in a home setting.
