Royal Health Care Services is seeking an experienced Home Health PRN Assessment Registered Nurse (RN) to perform comprehensive in-home clinical assessments for skilled nursing patients. This position is responsible for completing Start of Care (SOC), Resumption of Care (ROC) and Recertification (RECERT) assessments, including accurate completion of OASIS assessments.
The Assessment Nurse develops individualized Plans of Care, identifies patient needs and ensures documentation meets Medicare, Medicaid, AHCA, and agency standards. Responsibilities
- Demonstrated expertise conducting comprehensive in-home assessments for patients with complex medical, neurological, cognitive, and behavioral health conditions, including Alzheimer's disease and other dementias
- Assess physical, functional, cognitive, psychosocial, environmental, and safety needs. Develop individualized patient care plans based on physician orders, assessment findings, and patient goals
- Obtain and document vital signs, medication reconciliation, health history, and clinical findings
- Identify changes in patient condition and communicate significant findings to clinical leadership
- Educate patients and caregivers regarding diagnoses, medications, disease management, home safety
- Ensure compliance with Medicare Conditions of Participation, Florida AHCA regulations, CMS requirements, agency policies, and accepted nursing standards
Required Qualifications
- Current Florida Registered Nurse (RN) license in good standing
- Minimum 2 years of nursing experience
- Experience managing medically complex patients in the home setting
- Demonstrated experience in Electronic Health Record (EHR) documentation
- Strong clinical assessment and critical thinking skills
- Reliable transportation and valid driver's license. Ability to travel to client homes within Volusia and Flagler Counties
- Current CPR certification
Preferred Qualifications
- COS-C (Certificate for OASIS Specialist – Clinical) or equivalent OASIS certification
- Experience with Medicare-certified home health agencies
- Bilingual capabilities (Spanish/English)
Salary: Commensurate with experience
Pay: From $25.00 per hour
