The Clinical Practice Facilitator provides lateral integration of healthcare services through the design, implementation, and evaluation of patient care by coordinating, delegating and supervising the care provided by a healthcare team at the clinical level.
Job Requirements:
Required:
- Currently licensed as a Registered Professional Nurse in New York State.
- Master's degree in Nursing or equivalent Master's degree (i.e. MPH) or currently enrolled in an accredited Master's Degree program with a graduation date within three (3) years of hire.
- Basic Life Support (BLS) certification required at the time of hire or within two (2) weeks after hire.
- Neonatal Resuscitation (NRP) certification required at the time of hire or within two (2) weeks after hire.
- Five (5) years recent RN and/or NP experience in a Level 3 or Regional NICU.
Preferred: Board Certification as a Neonatal Nurse Practitioner. Nationally recognized certification as a Neonatal or Pediatric NP. Experience in a Clinical Nurse Specialist role
