LPN - Van Buren Health & Rehab
Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) working overnight shifts from 11 p.m. to 7 a.m., Monday through Friday, with weekend double shifts and Baylor pay. This dynamic role offers an exciting opportunity to provide compassionate, high-quality patient care in a supportive environment. As an integral part of our healthcare team, you will deliver essential nursing services, ensuring patient comfort and safety while collaborating with multidisciplinary teams. Your expertise will help us uphold the highest standards of clinical excellence and patient-centered care.
Duties
- Communicate with families and residents
- Provide supervision to staff performing ADL's
- DIrect communication with the MD if needed
- Follow up and document on any incidents/accidents
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration.
- Document all patient interactions meticulously within Electronic Health Record (EHR) PointClickCare, ensuring compliance.
- Collaborate with interdisciplinary teams on discharge planning, case management, and post-acute care strategies for diverse patient populations.
- Provide education on health topics such as nutrition, medication management, or disease-specific care plans like Alzheimer's or hospice care.
Requires an Active and Unrestricted LPN License
Pay: $30.00 - $31.00 per hour
