Position Summary
We’re seeking an enthusiastic RN Home Visitor to support our mission to help children achieve their highest potential. The Nurse-Family Partnership (NFP) RN Home Visitor is a licensed, registered nurse responsible for providing home visiting services to first-time, pregnant women and their families, following the evidence-based NFP model. The RN develops therapeutic relationships with clients and delivers structured, strengths-based interventions from pregnancy through the child’s second birthday to improve pregnancy outcomes, child health and development, and family self-sufficiency.
Work Requirements:
- Deliver in-home nurse visits to first-time mothers from early pregnancy through the child’s second birthday, following the Nurse-Family Partnership model.
- Conduct comprehensive health, psychosocial, and developmental assessments.
- Educate clients on prenatal health, breastfeeding, newborn care, postpartum recovery, child development, and parenting.
- Promote maternal self-efficacy, positive health behaviors, and family goal achievement.
- Establish and maintain a trusted, professional relationship with each client and her family.
- Engage clients using strength-based, client-centered communication strategies.
- Demonstrate cultural humility, empathy, and respect in all interactions.
- Document all client interactions and assessments accurately and in real time using NFP’s web-based data system.
- Monitor client outcomes and track service delivery to ensure fidelity to the NFP model.
- Maintain clinical records in compliance with HIPAA and agency standards.
- Collaborate with internal departments and external partners to connect families with behavioral health, medical, housing, food, and other supportive services.
- Participate in the agency’s centralized referral system to ensure timely and appropriate enrollment.
- Follow up on referrals and coordinate care to promote continuity and wraparound support.
- Engage in regular reflective supervision to enhance critical thinking, emotional resilience, and clinical insight.
- Participate in weekly team meetings and continuous quality improvement activities.
- Offer peer support and contribute to a positive, collaborative team culture.
- Complete all required NFP Core Education and ongoing training within designated timelines.
- Stay informed of current best practices in maternal-child health, home visiting, and public health nursing.
- Participate in professional development activities to advance skills and meet licensure requirements.
- Offer flexible scheduling, including occasional evening or weekend visits based on client needs and nurse discretion.
- Travel throughout home county and beyond if caseloads require (Dona Ana and Otero)

Qualifications & Skills
Minimum Qualifications:
- Active and unrestricted Registered Nurse (RN) license
- 2 years nursing experience in home visiting, maternal-child health, public health, or community-based care
- Strong clinical assessment skills
- Excellent communication and interpersonal skills
- Proficiency in electronic health records (EHR) and data reporting systems
- Microsoft Office programs proficiency
- Ability to work in a changing environment gathering relevant information to integrate a change to the process.
- Ability to use decision making skills to plan and accomplish goals without direct oversite.
Preferred Qualifications:
- CCM (Certification Case Manager)- CCMC national board certification
- Prior experience in NFP or other home visiting programs
- Strong knowledge of Medicaid, Managed Care Organizations (MCOs), and community health systems
- Bilingual skills English/Spanish are a plus.
PHYSICAL REQUIREMENTS / WORK ENVIRONMENT:
The ability to lift and carry objects weighing up to 40 pounds (e.g., boxes, equipment) without assistance. Capability to stand and walk for extended periods, including moving around the workspace, performing tasks, and interacting with colleagues or customers. Must be able to bend, stoop, crouch, and kneel to access various items, equipment, or workspaces as required. Aptitude to reach overhead, horizontally, and below waist level to retrieve and handle objects or materials. Capacity to perform physically demanding tasks continuously throughout the workday. Physical ability to move freely and quickly in various work environments, including navigating stairs, ramps, or uneven terrain if necessary. If the job involves driving, a valid driver's license, and the ability to operate vehicles safely and responsibly may be required.
