The RN Case Manager is responsible for providing nursing care to patients, including identifying and coordinating patient/family care with other members of the Interdisciplinary Team and health care providers. As an RN Case Manager, you will:
- Complete an initial assessment of patient and family to determine hospice care needs; provide a complete physical assessment and history of current and previous illness
- Use health assessment data to determine nursing diagnosis
- Make visits for symptom management and support as needed
- Modify patient treatment plans as indicated by patients' responses and conditions
- Maintain current, accurate, detailed reports in accordance with policy
- Monitor, record and report symptoms and changes in patients' condition to the IDG
- Provide health care instructions to the patient as appropriate per assessment and plan
- Obtain and receive physician orders as required
- Pronounce patients
- Triage calls and assist patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs
- Complete other duties as assigned
Requirements
- Current license to practice as a registered professional nurse in Georgia
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience preferred
- Good verbal and written communication and problem solving skills
- Responsible valid driver’s license; reliable, insured, automobile for making home visits required

Pay: From $85,000.00 per year
