Aventura at West Park is seeking a compassionate and motivated PRN Occupational Therapist (OT) to join our dynamic in-house rehabilitation team. If you're passionate about helping patients regain independence and improve their quality of life, we'd love to hear from you!
PRN (as-needed) schedule with flexible availability
In-house therapy program
Skilled Nursing & Long-Term Care setting
Collaborative interdisciplinary team
Supportive leadership and positive work environment
Opportunity to pick up additional hours as available
Evaluate residents and develop individualized treatment plans
Provide skilled occupational therapy services to improve functional independence
Collaborate with physicians, nursing staff, therapy team members, residents, and families
Maintain timely and accurate documentation in Net Health
Utilize PointClickCare (PCC) as part of the interdisciplinary care process
Ensure compliance with all state, federal, and company regulations
Current Ohio Occupational Therapist (OT) license in good standing
Graduate of an accredited Occupational Therapy program
Skilled nursing or long-term care experience preferred, but not required
Strong communication, organization, and clinical skills
Experience with Net Health and PointClickCare (PCC) is preferred
Flexible PRN scheduling
Collaborative in-house therapy model
Modern clinical documentation systems
