PCS is seeking an ID/DD Support Coordinator to provide support coordinationcase management services to adults with developmental/intellectual disabilities and assure that these individuals receive appropriate services, assisting them in accessing needed medical, psychiatric, educational, residential and other support services. A Bachelor's degree in Behavioral Sciences or related Human Services field and minimum of one-year experience working with the population is required. Basic computer skills are needed and experience with assessment and service plan development is preferred. Must have a valid driver's license, safe driving record, and reliable ability to transport self and others to worksites, trainings, and meetings. Salary is determined by educationexperience as it relates to the position.
