Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature; report changes promptly to nursing staff.
- Administer medications and perform basic medical tasks following home healthcare regulations and medical documentation standards.
- Support patients with memory care needs, dementia care, Alzheimer’s care, and hospice or end-of-life care with compassion and patience.
- Maintain patient confidentiality in accordance with HIPAA regulations while documenting medical records accurately within Electronic Health Record (EHR) systems.
- Assist with light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients.
- Facilitate patient transfers using appropriate techniques and equipment like wheelchairs or Hoyer lifts to promote safety during mobility assistance.
Skills
- Proven experience working with geriatric patients, individuals with developmental disabilities, or those requiring specialized dementia or Alzheimer’s care.
- Strong knowledge of medical terminology, medication administration protocols, and patient personal care assistance techniques.
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
- Ability to perform patient observation diligently and recognize signs of health deterioration or emergencies requiring immediate response.
- Competence in providing home health care assistance within regulatory guidelines while maintaining patient confidentiality under HIPAA standards.
- Excellent communication skills for effective client interaction and teamwork within healthcare settings.
- Certifications such as First Aid, Basic Life Support (BLS), CPR, and training in home health agency procedures are highly valued.
