Position: Systems Navigator
Primary Function: The Systems Navigator provides mental health services to clients of Northland Counseling Center, Inc. All activities as an employee of Northland Counseling Center, Inc. will be performed in an ethical, professional manner and in accordance with the policies and procedures of Northland Counseling Center, Inc. and will be subject to supervision and/or oversite.
Minimum qualifications: Valid driver’s license, insurance, reliable vehicle. Ability to complete and pass a background check. Certification or experience in one of the two following areas is mandatory:
Mental Health Practitioner (MHP) or Community Health Worker (CHW)

· A CHW certificate is received within the Minnesota State Colleges and Universities system.
Desired Qualifications – Experience in writing care plans, identifying goals, motivational interviewing, time management, crisis assessment and intervention is ideal. Knowledge of community resources and healthcare providers is strongly preferred. Applicants should have excellent communication skills and the ability to work well individually and as part of a team.

Roles/Responsibilities:
Manage a shared case load of up to 56 clients.
Complete DHS assessments within time restrictions to identify and evaluate goals.
Connect clients to resources to manage medical, social, and mental health conditions.
Assess for and provide culturally competent care to clients.
Provide flexible support to advocate for and assist client in achieving goals to attain optimal health.
Assist clients with setting up, preparing for, and attending (this may include going along with) appointments. Navigate the healthcare system and coordinate care to ensure follow-up.
Document according to NCC policy.
Complete ongoing training assigned by NCC or required by DHS.
Other duties as assigned by supervisor.

Develop and/or provide In-service training to staff as requested.
Maintains an active license status – for licensed professionals
Provide services and conduct self in a manner consistent with the professional Code of Ethics.
Assess or provide therapy, counseling, intervention and/or education to clients in a professional manner.
Provide clinically appropriate treatment based on mental health assessment and treatment plans.
Complete, timely and accurate documentation.
Participating in Clinical Supervision, as needed, by a board approved licensed provider
Participate in planned supervision and case consultation conferences.
Ability to maintain active caseload set forth by your direct supervisor.
Participate in open-access rotation.
Maintain compliance with needs
Attendance and/or completion of mandatory staff meetings and/or training
Ensure compliance with all agency license/certifications (Rule 29, CCBHC, program certifications), HIPPA, and other program(s) compliance and facility policies and procedures
Follow dress code as outlined in employee handbook or directed by supervisor
Follow proper reporting procedures as outlined in policies and procedures manual

Preferred Education and Experience
Previous experience working in mental health and/or substance use.
Previous experience working with clients who may have legal consequences.
Previous experience working with clients with dual diagnosis and/or SPMI diagnosis
Familiarity with various modalities of therapeutic interventions.
Experience in writing care plans, identifying SMART goals, motivational interviewing, time management, crisis assessment and intervention is ideal. Must be 18 years of age or older
Must be proficient in MS office and Internet applications
Must pass State and Federal Background Study, be clear of any predatory offenses
Must not be listed on the exclusion list to provide services to Medicaid and/or Medicare Recipients
Knowledge of and ability to perform basic evaluation of a person’s behavior in relation to mental health
Knowledge of mental health and substance use disorders
Knowledge of client rights and staff responsibilities
Knowledge of Maltreatment of Minors and Vulnerable Adult Acts
Knowledge of Behavioral Emergency Policy and Protective Procedures
