Clinical Care Associate
1. Access to care
a. Facilitate Rx patient assistance programs
2. Retention in Care
a. Notify social workers or office manager of reported barriers to care
b. Review of medications and adherence (including medications from other treating clinicians) and document in chart as part of rooming during clinic visit
c. Optional attendance at monthly Ministry Café
d. Patient education: adherence, nutrition, smoking cessation, etc
e. Self-management model to assist pts in goal setting to improve health
f. Fill medication boxes
g. Medication Crisis Intervention
h. Pursue patients re: GYN, dental & eye
3. Utilization/Quality of Care
- Medical triage, phone or drop-in
- Prepare charts for clinic
- Retrieve lab results
- Order prescriptions per physician orders
- Fulfill request for medication refills through EMR, verbal and forms per physician orders
- Fulfill all medical requests for medical records from medical facilities
- Keep physicians informed of patient issues/problems
- Assist Reviewing labs, add to chart, contact physician for abnormal results
- Obtain prior authorization for medication or procedures
- Track medication deliveries
- Patient triage/rooming during clinic, following established procedures
- Administer vaccines and medication, perform blood draws and HIV counseling
- Post-clinic review/audit of charts according to established protocol
- Dispense or phone in OTC or Rx medications per physician order
- Pay away patient medication in medicine room and keep room organized
- Assist Authorizing patient medication shipments
- Patient discharge following clinic visit, following established procedures
- Monitor required screenings
- Monitor expiration dates on CCC medication stock
- Request medical records for new pts
- Post clinic review of charts you handled, following established procedures
- Medical referrals and follow up
- Document in EMR and on billing sheet
- File Labs in patient’s charts
- After clinic, clean exam room
- Keep exam room stocked and notify Nurse Manager of needed supplies
- After-hours message retrieval and follow up
- Participate in Quality activities as part of interdisciplinary staff meetings
4. General
a. Clean/organize med room and lab
b. Monitor and change sharp containers
b. Maintain patient confidentiality
c. Maintain patient Documentation
i. Document patient contact
ii. Document pertinent information from other agencies/providers
d. Document/chart patient progress immediately and accurately
e. Perform work in a timely and productive manner
f. Take action to improve knowledge, skills and performance based on feedback or on self-identified needs; maintain licensure
g. Maintain daily log
h. Assist with Ministry fundraising when asked
i. Performs other duties as necessary
