A health services organization in New York City is looking to add a new Case Planner to join their growing team in Queens, supporting children and families involved in foster care through field-based case planning, family visits, and service coordination.
About the Opportunity:

Schedule: Monday-Friday

Hours: 9am to 5pm or 10am to 6pm or 11am to 7pm

Setting: Hybrid (90% field, 10% office)
Responsibilities:

Create service plans in partnership with children, biological families, and foster families to address safety, behavioral, educational, psychological, and permanency needs

Conduct regular casework contacts, document progress notes, and monitor safety, well-being, and case progress

Schedule and supervise mandated visits and complete field visits at homes, schools, courts, hospitals, and other required sites

Participate in supervision, family team conferences, and court hearings, providing updates and testimony as needed

Coordinate with integrated services and use case data to track outcomes, support services, and ensure timely documentation
Qualifications:

1+ year of Case Management experience; 1 year of Foster Care or Mental Health experience

Associate's and/or Bachelor's Degree

Experience in Child Welfare

Proficiency in Microsoft Office, including Outlook and Excel.

Ability to meet deadlines and work flexible hours, including some evenings as needed

Excellent written, oral, organizational, and interpersonal communication skills
Desired Skills:

Bilingual (English and Spanish)

Experience with Child Welfare Case Management systems
