Ideal Home Base: Candidates residing along the I-75 corridor between Macon and South Metro Atlanta are strongly preferred. Communities such as Forsyth, Barnesville, Jackson, Griffin, Locust Grove, McDonough, Stockbridge, and surrounding areas are ideal.
About the Position
Modani Care is seeking an experienced, compassionate Registered Nurse (RN) to join our growing clinical leadership team as a Regional Clinical RN.
This full-time, field-based position is responsible for managing the clinical needs of an assigned geographic region by completing admissions, annual assessments, resumptions of care, supervisory visits, and skilled nursing visits while providing clinical guidance and support to LPNs and caregivers.
This role is ideal for an RN who enjoys working independently, takes ownership of their responsibilities, and is committed to delivering exceptional client care while maintaining the highest standards of clinical excellence and regulatory compliance.
Visits are coordinated geographically whenever possible to maximize efficiency and reduce unnecessary travel.
Responsibilities
- Complete admissions, annual assessments, and resumptions of care (ROC)
- Perform supervisory visits in accordance with agency policy and state regulations
- Provide skilled nursing visits as ordered by the physician
- Independently manage an assigned regional caseload while ensuring all required visits are completed within regulatory timeframes
- Accurately document assessments, interventions, client education, and care provided in accordance with agency, state, and regulatory requirements
- Review client documentation for accuracy, completeness, and compliance
- Promptly communicate significant client condition changes, incidents, complaints, and physician notifications
- Provide clinical guidance, mentoring, and support to LPNs and caregivers under the direction of the Executive Clinical Director
- Assist in maintaining survey readiness and agency compliance
- Collaborate with office staff to coordinate client care and ensure timely completion of required visits
Qualifications
- Active, unrestricted Georgia Registered Nurse (RN) license
- Minimum of two (2) years of nursing experience
- Current CPR certification
- Current TB screening or test
- Valid driver's license
- Reliable transportation with active automobile insurance
- Strong assessment, organization, documentation, and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work independently while managing a regional field-based schedule
- Comfortable providing care to both adult and pediatric clients
- Home health, hospice, personal care, case management, pediatric, or community-based nursing experience preferred
