full-time, in-office position
Assist in the coordination of patient care, under the direction of the Clinical Manager, and Branch director. Answer phones, receive all intake information for paraprofessional cases, and enter the appropriate information, in the computer.
The Scheduler will complete intake forms and appropriately route them, for admission approval. Assist in the scheduling of appropriately skilled person to fill the case, input scheduling information on the computer, and continue to assist in the coordination of the scheduling. Schedule replacement workers, as needed, and input scheduling information. Take telephone referral information. Perform other duties, as directed. Enter patient information into computer, check Medicare eligibility. Communicate with Clinicians, Case Managers, families, physician offices or facilities.
Requirements:

- Current Driver's License with current auto insurance
- CEU on Elderly Abuse, Human Trafficking and Alzheimer's/Dementia
- TB test results
