Licensed Practical Nurse (LPN)Position Summary
The Licensed Practical Nurse (LPN) provides nursing care and support to residents living in an apartment-based home care setting. The LPN is responsible for delivering direct nursing services, administering medications and treatments, monitoring resident health, and participating in daily personal care. Working collaboratively with families, and the interdisciplinary team, the LPN promotes each resident's independence, dignity, safety, and highest level of functioning while ensuring compliance with all state and federal regulations.
Essential Duties and Responsibilities Resident Care
- Provide direct nursing care according to individualized service plans and physician orders.
- Participate in residents' daily care, including assistance with activities of daily living (ADLs) , such as bathing, dressing, grooming, toileting, transfers, mobility, and feeding.
- Monitor residents' physical, emotional, and cognitive status and promptly report changes in condition.
- Assess residents for signs of illness, injury, pain, or decline and implement appropriate nursing interventions.
- Respond appropriately to emergencies and provide first aid or other nursing care within the LPN scope of practice.
Medication Management
- Prepare, administer, and document medications safely and accurately following physician orders and agency policies.
- Monitor residents for medication effectiveness, adverse reactions, and side effects.
- Maintain accurate Medication Administration Records (MARs).
- Coordinate medication refills, pharmacy communication, and medication storage.
- Ensure controlled substances are properly counted, secured, and documented according to policy.
Treatments and Clinical Responsibilities
- Perform nursing treatments including wound care, dressing changes, catheter care, blood glucose monitoring, and other physician-ordered procedures.
- Obtain and document vital signs and other clinical observations.
- Collect specimens as ordered.
- Maintain infection prevention and control practices.
Documentation
- Complete accurate, timely, and thorough documentation of nursing care, medications, treatments, resident observations, incidents, and communication.
- Maintain confidential resident records in accordance with HIPAA regulations.
Safety and Compliance
- Follow all agency policies, state regulations, infection control standards, and professional nursing practices.
- Ensure a safe, clean, and therapeutic living environment.
- Report incidents, accidents, medication errors, and safety concerns according to agency policy.
- Participate in quality improvement activities and mandatory staff education.
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing.
- CPR certification (or ability to obtain upon hire).
- Experience in home care, assisted living, residential care, or long-term care preferred.
- Knowledge of medication administration, nursing documentation, and infection control practices.
- Strong assessment, communication, and organizational skills.
- Ability to work independently while functioning as part of a multidisciplinary team.
Physical Requirements
- Ability to stand, walk, bend, stoop, lift, transfer, and assist residents as needed.
- Ability to lift up to 50 pounds with or without assistance.
- Ability to respond quickly in emergency situations.
Work Environment
This position is performed in an apartment-based home care setting where residents live independently with varying levels of nursing and personal care support. The LPN provides both skilled nursing services and hands-on resident care to promote health, safety, comfort, and independence while maintaining a home-like environment.
Pay: $25.43 - $30.62 per hour
