Coral Reef Home Health Solutions is seeking a compassionate and motivated Physical Therapist (PT) to provide skilled home health services to private-duty patients throughout Broward County, Florida.
Responsibilities
* Perform comprehensive physical therapy evaluations and develop individualized plans of care.
* Provide skilled physical therapy interventions in accordance with the established treatment plan.
* Evaluate patients' functional abilities, mobility, strength, balance, and safety needs.
* Modify treatment plans based on patient progress and clinical findings.
* Educate patients and caregivers on therapeutic exercises, fall prevention, mobility, and home safety.
* Supervise Physical Therapist Assistants (PTAs) as required by Florida regulations and agency policies.
* Maintain accurate, timely, and complete clinical documentation.
* Collaborate with physicians and the interdisciplinary team to ensure quality patient care.
* Comply with all agency policies, procedures, and applicable state regulations.
Qualifications
* Current Florida Physical Therapist (PT) License.
* Current CPR Certification.
* Valid driver's license and reliable transportation.
* Professional liability insurance.
* Excellent communication, assessment, and organizational skills.
* Home health experience is preferred but not required.
* Ability to work independently and manage patient caseloads efficiently.
