Responsibilities
- Conduct comprehensive patient evaluations to determine functional limitations and develop tailored treatment plans.
- Implement therapeutic interventions including manual therapy, trigger point therapy, and exercise programs to promote recovery.
- Manage discharge planning processes by coordinating care transitions and providing patient education for ongoing self-care.
- Collaborate effectively with healthcare professionals such as physicians, nurses, occupational therapists, and social workers to optimize patient outcomes.
- Maintain accurate clinical documentation adhering to medical terminology standards and electronic health records (EHR) management protocols.
- Work with adult geriatric patients, and those requiring home & community care.
- Monitor patient progress regularly and adjust treatment strategies to ensure optimal rehabilitation results in outpatient or inpatient settings.
Qualifications
- Valid licensure or certification as a licensed Physical Therapist (PT) in the respective state.
- Proven experience practicing in a physical therapy role within skilled nursing facilities.
- Strong knowledge of anatomy, physiology, orthopedics, neurology, and geriatrics to inform effective treatment planning.
- Familiarity with clinical documentation standards and electronic health record (EHR/EMR) systems for accurate recordkeeping.
- Experience working with acute care settings and understanding procedures related to inpatient care in skilled nursing facilities.
- Ability to communicate clearly with adult patients while demonstrating empathy and professionalism.
- Knowledge of medical terminology relevant to physiotherapy practices such as trigger point therapy and rehabilitation techniques.

Pay: $40.00 - $53.00 per hour
