Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our home-based primary care team. This job will be three days in office and 2 days at home. This role requires a strong understanding of nursing practices, excellent communication skills, and the ability to work collaboratively within a healthcare setting.
Duties
- Telephonically manage patients' chronic medical conditions
- Telephonically support MDs and APRNs seeing patients in their homes and assisted livings.
- Telephonically support assisted living buildings with their patient needs
- Medication Management
- Durable medical Equipment Management
- Lab Support
- Maintain accurate patient records and documentation in accordance with healthcare standards.
- Collaborate with healthcare professionals to develop and implement patient care plans.
- Educate patients and their families on health management and wellness practices.
- Utilize knowledge of anatomy to provide comprehensive care tailored to individual patient needs.
- Referrals to community supports
- Referrals to specialists
Skills
- Strong knowledge of nursing standards and best practices in patient care.
- Strong assessment skills
- Strong knowledge of community resources.
- Home health care experience a plus
- Ability to communicate effectively with patients, families, and extended care teams.
- Ability to work autonomously and hit productivity standards.
- Excellent documentation skills.
Position is in office 8:30-5pm Monday through Friday. Productivity quotas. Must be willing to become licensed in other states, initially and candidate will have to become licensed in Connecticut after acceptance of job offer. Must have a private room to comply with HIPPA and Internet service to accommodate a work from home situation. Must have childcare on days that working from home as a full 8 hour day is expected.

Pay: $24.00 - $27.00 per hour
