Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using advanced EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks.
- Administer medications and injections accurately following aseptic techniques, including IV infusions, catheterizations, and medication management for acute and chronic conditions.
- Conduct diagnostic evaluations such as laboratory specimen collection, processing, and interpretation of results; utilize laboratory experience to support clinical decisions.
- Provide specialized care in areas such as critical care (ICU), emergency medicine, trauma medicine (Level I & II Trauma Center experience), neonatal resuscitation procedures, and pediatric or geriatric settings.
- Manage complex cases involving trauma, infectious diseases, hospice & palliative medicine, long-term care, memory care, behavioral health, or occupational health.
- Collaborate with physicians and other healthcare professionals to develop discharge plans, coordinate case management efforts, and ensure compliance with HIPAA and NCQA standards.
- Educate patients and families on health coaching topics including pain management, nutrition, wound care (including ostomy care), and post-acute care instructions.
Qualifications
- Valid RN license in good standing with current state requirements.
- Proven experience working with diverse populations including pediatrics, geriatrics (including dementia and Alzheimer's care), individuals with disabilities or developmental disabilities.
- Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth; familiarity with ICD-9/10 coding and medical documentation standards.
- Critical care experience preferred; including ICU or trauma center background (Level I & II Trauma Center experience).
- Skilled in advanced nursing procedures such as suturing, tube feeding management, ventilator support, SpO2 monitoring, phlebotomy, venipuncture, sterile processing techniques.
- Knowledge of medical terminology related to internal medicine, family planning, labor & delivery or outpatient clinics; experience working in hospital medicine or outpatient settings is highly desirable.
- Strong assessment skills with the ability to perform physical examinations and interpret physiology knowledge effectively.
- Ability to work efficiently in high-pressure environments such as emergency rooms or urgent care centers while maintaining compassionate patient-centered care.
- Excellent communication skills for triage assessments and patient education; familiarity with EMR/EHR systems like eClinicalWorks or Athenahealth is a plus.
